    posix_fallocate: Emulation fixes and documentation [BZ #15661]
    
    Handle signed integer overflow correctly.  Detect and reject O_APPEND.
    Document drawbacks of emulation.
    
    This does not completely address bug 15661, but improves the situation
    somewhat.
