Subject: Drop support for OpenSSL < 1.1.0

Delete code written to support OpenSSL < 1.1.0

Delete functions that have no effect in OpenSSL >= 1.1.0
 - ENGINE_load_builtin_engines()
 - OpenSSL_add_all_algorithms()
 - ERR_load_crypto_strings()
 - EC_KEY_set_asn1_flag(ecKey, OPENSSL_EC_NAMED_CURVE)

Switch AppVeyor to use pre-built OpenSSL 1.1.0
